Do you have Medicare patients that meet the below criteria?

• Has Diabetes Mellitus.
• Injects Insulin 3 or more times a day, or is on an insulin pump.
• Frequently has to adjust their insulin regimen on the basis of blood sugar testing results.
• And has had an in-person visit with you in the past 6 months to evaluate their diabetes control.

• Collecting and reviewing progress notes and scripts.
• Verifying patient insurance - we help streamline the insurance process by facilitating and confirming insurance coverage and billing.
• Handling all the paperwork - Medicare and commercial/private plans to help ensure greatest benefits are received.
• Ensuring speedy product to patient delivery - once we have the patients progress notes, completed script, and insurance information sent to us, we can process and ship the CGM system within 24 hours and deliver it via USPS priority mail 2-day shipping
• Automating monthly refill process.
